Physical Form- Required for Middle School athletes, all new students entering from out of state, and students entering Kindergarten, 6th or 11th grade.
Dental Form- For students entering Kindergarten, 3rd, or 7th grade.
* Sections 1-6 are required, and due before the first sports practice. Section 7 is for a second sport. Please email completed forms to
